Privacy Policy (HIPPA)

Appointment and Financial Policies
Scheduling Appointments . A deposit of $85 is required at the time of scheduling for any appointment 90 minutes in length or longer.

  For patients with insurance , a deposit of 50% is requested for non-routine appointments when full payment is not made at the time of service and insurance is to be submitted. As a courtesy, we will complete and send dental insurance forms for you; however, your insurance policy is a contract between you and your insurance company. If payment has not been received from your insurance company within 45 days, we will request payment in full from you. Any overpayment will be returned promptly. We expect payment in full within 15 days of insurance payment unless other arrangements have previously been made. While insurance benefits may be forthcoming, we are careful to make no assumptions about their amount. Knowing your insurance benefits is your responsibility; please contact your insurance company with any questions regarding coverage.

For patients without insurance , payment in full is requested at the time of service unless financing is arranged before treatment. Any insurance claims denied or remaining unpaid after 60 days will automatically become the responsibility of the patient.

For patients with or without insurance , we are more than happy to discuss easy financing and low monthly payments with you. All treatment plans are based upon clinical information available at the start of treatment. Treatment plan fees quoted are guaranteed for 90 days, however, changes in actual services provided may change the overall cost to you. Fees for biopsy will be billed by outside laboratories.

Missed & Rescheduled Appointments . There is a fee of $85 or 10% of the appointment cost, (whichever is greater) for missed appointments. This includes patients we are unable to see as scheduled due to tardiness, forgotten pre-medication, or forgotten appliances. This also includes changes in scheduled treatment, unless due to medical necessity. For example, changing an appointment scheduled for two crowns to only one at the time of the visit.

Cash and personal checks are accepted. Master Card, VISA, Discover, and American Express credit card payments are also welcome. If an extended payment plan is desired, please ask us about the National City Bank or Care Credit programs.

  Returned checks and balances older than 30 days may be subject to interest charges of 1.5% per month or service charges. Any attorney or collection fees incurred due to delinquency in payment will also be charged to the patient.
